Baxter, KY — Heartbreak has struck the Baxter community after a devastating collision at the intersection of US Highway 421 and US Highway 119 claimed two lives on the afternoon of August 13, 2025.
The community is in mourning following the death of Phyllis Frazier, who was killed in a serious car accident . The collision, described by authorities as severe, claimed her life and left friends, family, and neighbors reeling from the sudden tragedy.
Officials have confirmed her passing and notified her loved ones. Remembered for the kindness and impact she brought to those around her, Phyllis leaves behind a legacy of warmth and generosity that will not be forgotten.
According to the Harlan County Coroner’s Office, Deputy Coroner John Jones responded to the scene at 12:14 p.m. and pronounced both victims — Phyllis Frazier, of New Tazewell, Tennessee, and Franklin D. Saylor, Jr., of Baxter, Kentucky — deceased at 12:24 p.m. A third individual involved in the accident was transported to Harlan ARH Hospital for treatment.
Authorities say the Kentucky State Police and Harlan County Coroner’s Office are actively investigating the circumstances surrounding the crash. First responders from multiple agencies, including Lifeguard Ambulance, Harlan City Fire Department, Harlan County Rescue Squad, Harlan County Sheriff’s Office, Harlan City Police, and Sunshine Fire Department, worked tirelessly at the scene.
